What is hypogonadism?

Hypogonadism refers to a condition in which the sex glands produce little to no sex hormones. The condition can affect both biological males and females.

Causes

Hypogonadism has two basic types of causes:

• Primary hypogonadism - This means there is a problem with the gonads themselves, which include the testes in men and ovaries in women. Primary conditions can be inherited such as Turner syndrome in women. Or acquired later in life due to infection, trauma, toxins, blood flow issues, radiation treatment, chemotherapy drugs, autoimmune disorders, and metastatic cancers.
• Secondary hypogonadism - This is caused by a problem in the hypothalamus or pituitary gland in the brain. The hypothalamus and pituitary signals the gonads to produce hormones. If they malfunction, hormone levels can get too low. Causes include head injuries, tumors, infections, blood flow obstruction, iron overload, and certain medications.

Symptoms

Symptoms can vary depending on the age of onset. General symptoms in adult males may include:

• Decreased energy and stamina
• Depressed mood and poor concentration
• Reduced strength and endurance
• More body fat and less muscle mass
• Decrease in facial and body hair
• Gynecomastia or increase in breast size

Females may experience absent or irregular periods, reduced sex drive, hot flashes, and fertility issues.

Children with hypogonadism may experience delayed or absent puberty and abnormal growth patterns.

Diagnosis and Treatment

Doctors use blood tests to measure hormone levels and examine the pituitary and gonads. Treatment depends on the cause, but may include hormone replacement therapy to get levels back to normal.
